By signing this consent form, I understand and agree to the following:

• Story Content: The story may be shared in written, audio, or video format, as submitted by me or my child.
• Purpose: The purpose of sharing this story is to inspire and motivate other children who may face similar challenges, and to promote the message of resilience and determination.
• Rights: I understand that Dr. Rinat Green and Kol Koreh have the right to edit or modify the story (while preserving its core message) for clarity or length, if necessary.
• Usage: The story may be used for promotional and educational purposes across various platforms, including social media, the Kol Koreh website, and Dr. Rinat Green’s Amazon Author Page.
• Privacy: My child's personal information (such as full name or identifying details) will not be disclosed without further explicit consent. Should my child’s story be shared anonymously or with a pseudonym, I will indicate that preference below.

I acknowledge that I am the legal parent/guardian of the child and have the authority to grant this permission.
